Lydia Dennehy is the resource specialist and a parent partner for Oregon Family to Family Health Information Center (OR F2F HIC). The OR F2F HIC holds a federal grant and is housed as part of the Oregon Center for Children and Youth with Special Health Needs. (OCCYSHN) Lydia is the parent of a transition age youth who was on an IFSP and IEPs throughout their school career.
She has served on various state advisory boards as a parent or consumer representative. Currently positions include the School Age Advisory Council, the Early Hearing and Detection and Intervention advisory committee, the Beneficiary Advisory Council, and the Medicaid Advisory Committee. She is committed to serving all Oregonians changing her career from education and community engagement to ten years of family peer support.
Lydia is a long time Oregonian having grown up on the South Coast of Oregon, gotten specialty healthcare in Eugene, and graduated from Ashland High School and Western Oregon University. She and her family currently live in Portland.

Using IEP’s and Transition Plans to Empower Youth Moving from Pediatric to Adult Health Care
This session will review background on what we know about youth and the transition to adult healthcare and then talk about specific tools that you can use with your students and clients to support them in becoming their own health care managers, advocates and create adult healthcare teams.
We will share specific skills, goals, and tools, you can incorporate into IEP’s and Transition plans. We will also be talking about how to engage youth and motivate them to build their skills and confidence.
We know that schools are talking with youth about educational changes but we want to remind youth that their health impacts, attendance at school and work, and their daily quality of life. We also talk about the roles of the team that is supporting the students, school nurses, para educators, peers, teachers, SLPs, OT’s, parents, and more. Each person can support these goals and build the skills necessary to promote healthy adult lives.
